This online learning program has been developed through a collaboration between the Ontario Pharmacists Association and the Institute for Safe Medication Practices Canada (ISMP Canada) It is designed for community pharmacists, pharmacy technicians and pharmacy assistants in Ontario.

The program provides an overview of safety principles and 2 important tools for improving patient safety – incident analysis and proactive risk assessment. The program also highlights aspects of the Ontario College of Pharmacists (OCP) medication safety quality assurance program, AIMS.

How will this knowledge help you in your practice? You will understand the basic principles required to improve patient safety, including proactively developing safety plans before things go wrong. You’ll also learn how to analyze errors that occur in the community pharmacy and develop strategies to prevent recurrences in the future. This learning program will help you to focus on identifying and preventing system-based problems and developing a positive safety culture to enhance the care you provide to patients.

Learning objectives:

  • Describe the context for patient safety in Canada and recognize and apply basic human factors engineering principles.
  • Determine when a system-based analysis is appropriate and prepare an incident timeline to support analysis.
  • Understand what Failure Mode and Effects Analysis (FMEA) is and how it can be used to improve patient safety.
