Accidents can happen at any time, to anyone. If an accident happened to you, it could mean adjusting to a physical disability, changing careers, modifying your home, changing your occupation, or needing long-term care. Often, the personal grief following a tragic accident is magnified by sudden and severe financial consequences.
While your personal loss is irreparable, you can protect yourself and your family from financial hardship with single or family plan accidental death and dismemberment (AD&D) coverage under the Association’s program.
Coverage is available in units of $25,000 to a maximum of $250,000. Specialized benefits offer payments to help you deal with various situations, and financial assistance is also provided to help you start on a new career path should you be unable to practice pharmacy.
In the event that an accident causes paralysis (quadriplegia, paraplegia, or hemiplegia), members will receive twice the amount of the principal sum insured. Also, in the case of long term total disability, where the insured has been approved for Waiver of Premium under OPA’s health and dental plan, the voluntary AD&D premium will be similarly waived (applicable only to members insured under the Health and Dental Plus plan).
If you choose the dependants’ coverage, eligible dependants will automatically be insured as follows:
- spouse without eligible children (60 per cent)
- spouse with eligible children (50 per cent)
- each child without spousal coverage (20 per cent)
- each child with spousal coverage (10 per cent).
Specialized benefits for single and family coverage include:
- spousal retraining
- exposure and disappearance
- family transportation
- special education.

This represents a simplified summary of the AD&D insurance plan. The actual terms and conditions of the insurance contract and the coverage are determined by the Master Policy, which is kept at the OPA office.
How to Apply?
Download the application for insurance, and complete the form:
- Section 1: Complete in full
- Section 2: Complete 2.1 and 2.2
- Section 8: Sign and return
- Section 9: To be retained by the proposed insured
Non-pharmacist/non-technician employees must have their application signed by the owner/sponsoring pharmacist.
Mail the original copy of your completed application to:

Faxed, scanned, or photocopied applications will not be accepted.
Do not send premium payments with your application. An invoice will be mailed with your coverage documents.
Within five business days of receipt of your application, the Association will mail you the certificate of insurance, details of the coverage, and an invoice for the initial premium.
