If either of the reporting conditions outlined below apply to you, you are required to complete the Incident Reporting form and send it, along with any claim or OCP investigation correspondence, to Adelaide Marquardt, HUB International HKMB, at [email protected]. If these conditions do not apply to you, you do not need to take any action and may disregard this letter.
Reporting conditions:
- If any claim has been made or a suit brought against you, in the 2023 calendar year, on account of any actual or alleged malpractice, error or mistake.
- If any provincial regulatory college or society established under a provincial act regulating your professional services, notified you in writing that investigators have been appointed by the Registrar of the College to inquire into and examine your practice, conduct or actions with respect to possible professional misconduct or incompetence.
