In July 2016, program enhancements included standardized documentation and tools introduced by the Ministry of Health and Long-Term Care (with formal implementation expected across all Ontario pharmacies by October 1, 2016.) Determining Patient Eligibility

MedsCheck Annual | Taking 3 or more chronic prescription medications. |

MedsCheck Follow-up | For patients that have received a MedsCheck Annual and require a follow-up based on the following criteria:

MedsCheck for Diabetes | Must be diagnosed with type 1 or type 2 diabetes and taking a medication for their condition |
Note: If a patient is diagnosed with diabetes and has received a MedsCheck Annual within the previous 12 months; this patient is eligible for a MedsCheck for Diabetes |
MedsCheck for Diabetes Follow-up | For patients that have received MedsCheck for Diabetes at the same pharmacy and require a follow-up consultation for purpose of:
