Deprescribing is a process of medication review and withdrawal of medications that are no longer necessary or may be causing harm. Deprescribing aims to reduce the medication burden and optimize the medication regimen to improve the patient’s health outcomes and quality of life. This course is designed for pharmacy professionals to understand the concept of medication deprescribing and its importance in optimizing the medication regimen of older adults. The course will cover the rationale behind medication deprescribing, the process of deprescribing, and the identification of potentially inappropriate medications (PIM) in older adults. The course will also cover deprescribing strategies to reduce, replace, or withdraw inappropriate medications.
This module is extracted from the Board Certified Geriatric Pharmacist (BCGP) Preparation Course. The content in this module provides current treatment guidelines for all populations and are not necessarily specific to geriatric care.
