I remember an interaction with one of my diabetic patient, his blood sugar and blood pressure was really high, his family physician sent him to endocrinologist to manage his diabetes, he gave him SGLT2 medication. Patient took it for few days then discontinued it due to side effects, he gave up in reaching the blood glucose target, decided to live with high blood sugar and high blood pressure.

I started helping the patient by adjusting his medications, with the permission of his family physician, I added Lantus at night and asked patient to go back on SGLT2 medication and managed the side effects that made him discontinued the medication, I recommended increasing the dose for his blood pressure medications, his A1C reached 7 after it was 8.5 his blood pressure started to be 128/79 after being at 167/94. Patient appreciated my help and was very happy reaching target and feeling good. I’m proud with the impact I have in my patient healthcare.
